The curved planar reformation (CPR) method re-samples the vascular structures along the vessel centerline to generate longitudinal cross-section views. The CPR technique has been commonly used in coronary CTA workstation to facilitate radiologists’ visual assessment of coronary diseases, but has not yet been used for pulmonary vessel analysis in CTPA due to the complicated tree structures and the vast network of pulmonary vasculature. In this study, a new curved planar reformation and optimal path tracing (CROP) method was developed to facilitate feature extraction and false positive (FP) reduction and improve our PE detection system. PE candidates are first identified in the segmented pulmonary vessels at prescreening. Based on Dijkstra’s algorithm, the optimal path (OP) is traced from the pulmonary trunk bifurcation point to each PE candidate. The traced vessel is then straightened and a reformatted volume is generated using CPR. Eleven new features that characterize the intensity, gradient, and topology are extracted from the PE candidate in the CPR volume and combined with the previously developed 9 features to form a new feature space for FP classification. With IRB approval, CTPA of 59 PE cases were retrospectively collected from our patient files (UM set) and 69 PE cases from the PIOPED II data set with access permission. 595 and 800 PEs were manually marked by experienced radiologists as reference standard for the UM and PIOPED set, respectively. At a test sensitivity of 80%, the average FP rate was improved from 18.9 to 11.9 FPs/case with the new method for the PIOPED set when the UM set was used for training. The FP rate was improved from 22.6 to 14.2 FPs/case for the UM set when the PIOPED set was used for training. Back-propagation neural networks (BPNs) are traditionally trained using error measures such as sum-of-squares or cross-entropy. If the training sample size is small, and the neural network has a large number of hidden layer nodes, the BPN may be overtrained, i.e., it may fit the training data well, but may generalize poorly to independent test data. In this study, we investigated a training technique that maximized the approximate area under the ROC curve (AUC) to reduce overtraining. In general, the non-parametric AUC is a discontinuous and non-differentiable function of the neural network output, which makes it unsuitable for gradient descent algorithms such as back-propagation. We used a semidifferentiable approximation to AUC, which appeared to provide reasonable training for the data sets explored in this study. We performed a simulation study using synthetic data sets consisting of Gaussian mixtures to investigate the behavior of this new technique with respect to overtraining. Our results indicated that an artificial neural network trained using the AUC-maximization method is less prone to overtraining. The advantage of the AUC-maximization method was consistently observed over different values of hidden layer BPN nodes, training sample sizes, and the dimensionality of the feature spaces evaluated in our simulation study. For a five-hidden-node BPN trained using 50 training samples per class, the average test AUC was 0.896 (standard deviation (SD): 0.026) with AUC-maximization and 0.856 (SD: 0.028) with the sum-of-squares method. The gain in test performance by the AUC-maximization method over the traditional BPN training was greater when the training sample size was smaller. We also applied this new method to a data set previously acquired for characterization of masses on breast ultrasound as malignant or benign. Our results with this real-world data set had the same trend as with our simulation data sets in that the AUC-maximization technique was less prone to overtraining than the sum-of-squares method.

Vessel segmentation is a fundamental step in an automated pulmonary embolism (PE) detection system. The purpose of this study is to improve the segmentation scheme for pulmonary vessels affected by PE and other lung diseases. We have developed a multiscale hierarchical vessel enhancement and segmentation (MHES) method for pulmonary vessel tree extraction based on the analysis of eigenvalues of Hessian matrices. However, it is difficult to segment the pulmonary vessels accurately under suboptimal conditions, such as vessels occluded by PEs, surrounded by lymphoid tissues or lung diseases, and crossing with other vessels. In this study, we developed a new vessel refinement method utilizing curved planar reformation (CPR) technique combined with optimal path finding method (MHES-CROP). The MHES segmented vessels straightened in the CPR volume was refined using adaptive gray level thresholding where the local threshold was obtained from least-square estimation of a spline curve fitted to the gray levels of the vessel along the straightened volume. An optimal path finding method based on Dijkstra's algorithm was finally used to trace the correct path for the vessel of interest. Two and eight CTPA scans were randomly selected as training and test data sets, respectively. Forty volumes of interest (VOIs) containing "representative" vessels were manually segmented by a radiologist experienced in CTPA interpretation and used as reference standard. The results show that, for the 32 test VOIs, the average percentage volume error relative to the reference standard was improved from 32.9±10.2% using the MHES method to 9.9±7.9% using the MHES-CROP method. The accuracy of vessel segmentation was improved significantly (p<0.05). The intraclass correlation coefficient (ICC) of the segmented vessel volume between the automated segmentation and the reference standard was improved from 0.919 to 0.988. Quantitative comparison of the MHES method and the MHES-CROP method with the reference standard was also evaluated by the Bland-Altman plot. This preliminary study indicates that the MHES-CROP method has the potential to improve PE detection.

The ultimate goal of this project is to investigate whether the effect of a computer-aided detection (CAD) system on readers' performance (especially, in situation of an upgrade of the CAD system, or between two different CAD systems with similar design) can be accurately predicted without having to perform a multi-reader multi-case (MRMC) observer study and, if such prediction is possible, to establish the underlying methodology. Our current study is intended to provide evidence that would substantiate efforts toward such investigation. The objectives of this study were 1) to investigate the relationship between the number of radiologists reading a dataset of thoracic computed tomography (CT) images to identify lung nodules and the number of distinct findings and 2) to determine the number of readers needed to identify almost all clinically distinct findings in a dataset. We used data from a multi-reader multi-case (MRMC) observer study that consisted of six radiologists interpreting 85 thoracic CT examinations. To further illustrate our approach, we also utilized simulated data consisting of twelve readers interpreting 198 samples equally distributed between three levels of detection difficulty. For each possible reader grouping, the number of distinct findings identified by the readers in the group was calculated. Five types of regression models used to describe the relationship between the average number of distinct findings per case and the number of readers needed were compared. The result showed that the logistic model best fitted both the thoracic CT data and the simulated data. Our assumption is that adding more readers after a certain reader set size would mostly add redundant findings and, therefore, the benefit would be negligible. Using this model, the predicted number of readers was found to depend on the type of findings considered. Our study showed that the number of clinically distinct findings that can be identified by radiologists on CT lung examinations without the use of a CAD system may be limited and that identifying almost all of these findings may only require a limited number of readers.

A computerized system for segmenting lesions in head and neck CT scans was developed to assist radiologists in estimation of the response to treatment of malignant lesions. The system performs 3D segmentation based on a level set model and uses as input an approximate bounding box for the lesion of interest. We investigated the effect of the interobserver variability of radiologists' marking of the bounding box on the automatic segmentation performance. In this preliminary study, CT scans from a pre-treatment exam and a post one-cycle chemotherapy exam of 34 patients with primary site head and neck neoplasms were used. For each tumor, an experienced radiologist marked the lesion with a bounding box and provided a reference standard by outlining the full 3D contour on both the pre- and post treatment scans. A second radiologist independently marked each tumor again with another bounding box. The correlation between the automatic and manual estimates for both the pre-to-post-treatment volume change and the percent volume change was r=0.95. Based on the bounding boxes by the second radiologist, the correlation between the automatic and manual estimate for the pre-to-post-treatment volume change was r=0.89 and for the percent volume change was r=0.91. The correlation for the automatic estimates obtained from the bounding boxes by the two radiologists was as follows: (1) pretreatment volume r=0.92, (2) post-treatment volume r=0.88, (3) pre-to-post-treatment change r=0.89 and (4) percent preto- post-treatment change r=0.90. The difference between the automatic estimates based on the two sets of bounding boxes did not achieve statistical significance for any of the estimates (p>0.29). The preliminary results indicate that the automated segmentation system can reliably estimate tumor size change in response to treatment relative to radiologist's hand segmentation as reference standard, and that the performance was robust against inter-observer variability in marking the input bounding boxes.

Coronary CT angiography (cCTA) has been reported to be an effective means for diagnosis of coronary artery disease. We are investigating the feasibility of developing a computer-aided detection (CADe) system to assist radiologists in detection of non-calcified plaques in coronary arteries in ECG-gated cCTA scans. In this study, we developed a prototype vessel segmentation and tracking method to extract the coronary arterial trees which will define the search space for plaque detection. Vascular structures are first enhanced by 3D multi-scale filtering and analysis of the eigenvalues of Hessian matrices using a vessel enhancement response function specifically designed for coronary arteries. The enhanced vascular structures are then segmented by an EM estimation method. The segmented coronary arteries are tracked using a 3D dynamic balloon tracking (DBT) method. For this preliminary study, two starting seed points were manually identified at the origins of the left and right coronary artery (LCA and RCA). The DBT method automatically moves a sphere along the vessel whose diameter is adjusted dynamically based on the local vessel size, tracks the vessels, and identifies its branches to generate the left and right coronary arterial trees. The algorithm was applied to 20 cCTA scans that contained various degrees of coronary artery diseases. To evaluate the performance of vessel segmentation and tracking, the rendered volume of coronary arteries tracked by our algorithm was displayed on a PC, placed next to a GE Advantage workstation on which the coronary arterial trees tracked by the GE software and the original cCTA scan were displayed. Two experienced thoracic radiologists visually examined the coronary arteries on the cCTA scan and the segmented vessels to count untracked false-negative (FN) segments and false positives (FPs). The comparison was made by radiologists' visual judgment because the digital files for the segmented vessels were not accessible on the commercial system. A total of 19 and 38 artery segments were identified to be FNs, and 23 FPs and 20 FPs were found in the coronary trees tracked by our algorithm and the GE software, respectively. The preliminary results demonstrated the feasibility of our approach.

The purpose of this study was to develop and evaluate computer-extracted features for characterizing mammographic mass margins according to BI-RADS spiculated and circumscribed categories. The mass was automatically segmented using an active contour model. A spiculation measure for a pixel on the mass boundary was defined by using the angular difference between the image gradient vector and the normal to the mass, averaged over pixels in a spiculation search region. For the circumscribed margin feature, the angular difference between the principal eigenvector of the Hessian matrix and the normal to the mass was estimated in a band of pixels centered at each point on the boundary, and the feature was extracted from the resulting profile along the boundary. Three MQSA radiologists provided BI-RADS margin ratings for a data set of 198 regions of interest containing breast masses. The features were evaluated with respect to the individual radiologists' characterization using receiver operating characteristic (ROC) analysis, as well as with respect to that from the majority rule, in which a mass was labeled as spiculated (circumscribed) if it was characterized as such by 2 or 3 radiologists, and non-spiculated (non-circumscribed) otherwise. We also investigated the performance of the features for consensus masses, defined as those labeled as spiculated (circumscribed) or nonspiculated (non-circumscribed) by all three radiologists. When masses were labeled according to radiologists R1, R2, and R3 individually, the spiculation feature had an area A_z under the ROC curve of 0.90±0.04, 0.90±0.03, 0.88±0.03, respectively, while the circumscribed margin feature had an A_z value of 0.77±0.04, 0.74±0.04, and 0.80±0.03, respectively. When masses were labeled according to the majority rule, the A_z values for the spiculation and the circumscribed margin features were 0.92±0.03 and 0.80±±0.03, respectively. When only the consensus masses were considered, the A_z values for the spiculation and the circumscribed margin features were 0.96±0.04 and 0.87±0.04, respectively. We conclude that the newly developed features had high accuracy for characterizing mass margins according to BI-RADS descriptors.

Breast vascular calcifications (BVCs) are calcifications that line the blood vessel walls in the breast and appear as parallel or tubular tracks on mammograms. BVC is one of the major causes of the false positive (FP) marks from computer-aided detection (CADe) systems for screening mammography. With the detection of BVCs and the calcified vessels identified, these FP clusters can be excluded. Moreover, recent studies reported the increasing interests in the correlation between mammographically visible BVCs and the risk of coronary artery diseases. In this study, we developed an automated BVC detection method based on microcalcification prescreening and a new k-segments clustering algorithm. The mammogram is first processed with a difference-image filtering technique designed to enhance calcifications. The calcification candidates are selected by an iterative process that combines global thresholding and local thresholding. A new k-segments clustering algorithm is then used to find a set of line segments that may be caused by the presence of calcified vessels. A linear discriminant analysis (LDA) classifier was designed to reduce false segments that are not associated with BVCs. Four features for each segment selected with stepwise feature selection were used for this LDA classification. Finally, the neighboring segments were linked and dilated with morphological dilation to cover the regions of calcified vessels. A data set of 16 FFDM cases with vascular calcifications was collected for this preliminary study. Our preliminary result demonstrated that breast vascular calcifications can be accurately detected and the calcified vessels identified. It was found that the automated method can achieve a detection sensitivity of 65%, 70%, and 75% at 6.1 mm, 8.4 mm, and 12.6mm FP segments/image, respectively, without any true clustered microcalcifications being falsely marked. Further work is underway to improve this method and to incorporate it into our FFDM CADe system.

Segmentation of masses is the first step in most computer-aided diagnosis (CAD) systems for characterization of breast masses as malignant or benign. In this study, we designed an automated method for segmentation of masses on ultrasound (US) images. The method automatically estimated an initial contour based on a manually-identified point approximately at the mass center. A two-stage active contour (AC) method iteratively refined the initial contour and performed self-examination and correction on the segmentation result. To evaluate our method, we compared it with manual segmentation by an experienced radiologists (R1) on a data set of 226 US images containing biopsy-proven masses from 121 patients (44 malignant and 77 benign). Four performance measures were used to evaluate the segmentation accuracy; two measures were related to the overlap between the computer and radiologist segmentation, and two were related to the area difference between the two segmentation results. To compare the difference between the segmentation results by the computer and R1 to inter-observer variation, a second radiologist (R2) also manually segmented all masses. The two overlap measures between the segmentation results by the computer and R1 were 0.87+ 0.16 and 0.73+ 0.17 respectively, indicating a high agreement. However, the segmentation results between two radiologists were more consistent. To evaluate the effect of the segmentation method on classification accuracy, three feature spaces were formed by extracting texture, width-to-height, and posterior shadowing features using the computer segmentation, R1's manual segmentation, and R2's manual segmentation. A linear discriminant analysis classifier using stepwise feature selection was tested and trained by a leave-one-case-out method to characterize the masses as malignant or benign. For case-based classification, the area A_z under the test receiver operating characteristic (ROC) curve was 0.90±0.03, 0.87±0.03 and 0.87±0.03 for the feature sets based on computer segmentation, R1's manual segmentation, and R2's manual segmentation, respectively.

We are developing a computerized technique to reduce intra- and interplane ghosting artifacts caused by high-contrast objects such as dense microcalcifications (MCs) or metal markers on the reconstructed slices of digital tomosynthesis mammography (DTM). In this study, we designed a constrained iterative artifact reduction method based on a priori 3D information of individual MCs. We first segmented individual MCs on projection views (PVs) using an automated MC detection system. The centroid and the contrast profile of the individual MCs in the 3D breast volume were estimated from the backprojection of the segmented individual MCs on high-resolution (0.1 mm isotropic voxel size) reconstructed DTM slices. An isolated volume of interest (VOI) containing one or a few MCs is then modeled as a high-contrast object embedded in a local homogeneous background. A shift-variant 3D impulse response matrix (IRM) of the projection-reconstruction (PR) system for the extracted VOI was calculated using the DTM geometry and the reconstruction algorithm. The PR system for this VOI is characterized by a system of linear equations. A constrained iterative method was used to solve these equations for the effective linear attenuation coefficients (eLACs) within the isolated VOI. Spatial constraint and positivity constraint were used in this method. Finally, the intra- and interplane artifacts on the whole breast volume resulting from the MC were calculated using the corresponding impulse responses and subsequently subtracted from the original reconstructed slices. The performance of our artifact-reduction method was evaluated using a computer-simulated MC phantom, as well as phantom images and patient DTMs obtained with IRB approval. A GE prototype DTM system that acquires 21 PVs in 3º increments over a ±30º range was used for image acquisition in this study. For the computer-simulated MC phantom, the eLACs can be estimated accurately, thus the interplane artifacts were effectively removed. For MCs in phantom and patient DTMs, our method reduced the artifacts but also created small over-corrected areas in some cases. Potential reasons for this may include: the simplified mathematical modeling of the forward projection process, and the amplified noise in the solution of the system of linear equations.

We are developing a computer-aided detection (CAD) system for lung nodules in thoracic CT volumes. Our CAD system includes an adaptive 3D pre-screening algorithm to segment suspicious objects, and a false-positive (FP) reduction stage to classify the segmented objects as true nodules or normal lung structures. We found that the effectiveness of the FP reduction stage was limited by the different characteristics of the objects in the internal and the juxta-pleural (JP) regions. The purpose of this study was to evaluate object characteristics in the internal and JP regions of a lung CT scan, and to develop different FP reduction classifiers for JP and internal objects. Our FP reduction technique utilized shape, grayscale, and gradient features, as well as the scores of a newly-developed neural network trained on the eigenvalues of the Hessian matrix in a volume of interest containing the suspicious object. We designed an algorithm to automatically label the objects as internal or JP. Based on a training set of 75 CT scans containing internal and JP nodules, two FP classifiers were trained separately for objects in the two types of lung regions. The system performance was evaluated on an independent test set of 27 low dose screening scans. An experienced chest radiologist identified 64 solid nodules (mean diameter: 5.3 mm, range: 3.0-12.9 mm) on the test cases, of which 33 were internal and 31 were JP. Our adaptive 3D prescreening algorithm detected 28 internal and 29 JP nodules. At 80% sensitivity, the average number of FPs was 3.9 and 9.7 in the internal and JP regions per scan, respectively. In comparison, a classifier designed to work on both types of nodules had an average of 29.4 FPs per scan at the same sensitivity. Our results indicate that it is more effective to use two different classifiers for JP and internal nodules because of their different characteristics. FPs in the JP region were more difficult to distinguish from true nodules. Further investigation of task-specific FP reduction techniques is needed.

We are developing new techniques to improve the performance of our computer-aided detection (CAD) system for clustered microcalcifications on full-field digital mammograms (FFDMs). In this study, we designed an information fusion scheme by using joint two-view information on craniocaudal (CC) and mediolateral-oblique (MLO) views. After cluster candidates were detected using a single-view detection technique, candidates on CC and MLO views were paired using their geometrical information. Candidate pairs were classified as true and false pairs with a similarity classifier that used the joint information from both views. Each cluster candidate was also characterized by its single-view features. The outputs of the similarity classifier and the single-view classifier were fused and the cluster candidate was classified as a true microcalcification cluster or a false-positive (FP) using the fused two-view information. A data set of 192 FFDM images was collected from 96 patients at the University of Michigan. All patients had two mammographic views. This data set contained 96 microcalcification clusters, of which 28 clusters were proven by biopsy to be malignant and 68 were proven to be benign. For training and testing the classifiers, the data set was partitioned into two independent subsets with the malignant cases equally distributed to the two subsets. One subset was used for training and the other subset was used for testing. We compared three computerized methods for geometrically pairing cluster candidates on two mammographic views. The areas under the fitted ROC curves were 0.75±0.01, 0.74±0.01, and 0.76±0.01 for the three methods, respectively. The difference between any two methods measured by the area under the fitted ROC curve, A_z, was not statistically significant (p > 0.05). We also evaluated a new hybrid pairing scheme that used two different sensitivity levels for defining cluster pairs based on the single-view scores. The single-view CAD system achieved cluster-based sensitivities of 75%, 80%, and 85% at 0.48, 0.86, and 1.05 FPs/image, respectively. The joint two-view CAD system achieved the same sensitivity levels at 0.29, 0.46, and 0.89 FPs/image. When the hybrid pairing was used in the joint two-view CAD system, the same cluster-based sensitivities were achieved at 0.26, 0.37, and 0.88 FPs/image. Our results indicate that correspondence of cluster candidates on two different views provides valuable additional information for distinguishing FPs from true microcalcification clusters.

Many computer-aided diagnostic problems involve three-class classification (e.g., abnormal (malignant), normal, and benign classes). A general treatment of the performance of a 3-class classifier results in a complex sixdimensional (6D) ROC space for which no analysis tools are available at present. A practical paradigm for performance analysis and a figure of merit (FOM) are yet to be developed. The area Az under the ROC curve plays an important role in the evaluation of two-class classifiers. An analogous FOM for three-class classification is highly desirable. We have been investigating conditions for reducing the 6D ROC space to 3D by constraining the utilities of some of the decisions in the classification task. These assumptions lead to a 3D ROC space in which the true-positive fraction (TPF) of one class can be expressed as a function of the two types of false-positive fractions (FPFs) from the other two classes. In this study, we investigated the properties of the 3D ROC surface for a maximum-likelihood classifier under the condition that the three class distributions in the feature space are multivariate normal with equal covariance matrices. Under the same conditions, we studied the properties of the normalized volume under the surface (NVUS) that was previously proposed as an FOM for a 3-class classifier. Under the equal covariance Gaussian assumption, it can be shown that the probability density functions of the decision variables follow a bivariate log-normal distribution. By considering these probability density functions, we can express the TPF in terms of the FPFs, and numerically integrate over the domain of the TPF to obtain the NVUS. We have compared the NVUS value obtained by directly integrating the probability density functions to that obtained from a Monte Carlo simulation study in which the 3D ROC surface was generated by empirical "optimal" classification of case samples in the multi-dimensional feature space following the assumed distributions. The NVUS values obtained from the direct integration method and the Monte Carlo simulation were found to be in good agreement. Our results indicate that the NVUS exhibits many of the intuitive properties that a proper FOM should satisfy and may be used as a performance index under the conditions that we imposed on the utilities.

In computer-aided detection (CAD) applications, an important step is to design a classifier for the differentiation of the abnormal from the normal structures. We have previously developed a stepwise linear discriminant analysis (LDA) method with simplex optimization for this purpose. In this study, our goal was to investigate the performance of a regularized discriminant analysis (RDA) classifier in combination with a feature selection method for classification of the masses and normal tissues detected on full field digital mammograms (FFDM). The feature selection scheme combined a forward stepwise feature selection process and a backward stepwise feature elimination process to obtain the best feature subset. An RDA classifier and an LDA classifier in combination with this new feature selection method were compared to an LDA classifier with stepwise feature selection. A data set of 130 patients containing 260 mammograms with 130 biopsy-proven masses was used. All cases had two mammographic views. The true locations of the masses were identified by experienced radiologists. To evaluate the performance of the classifiers, we randomly divided the data set into two independent sets of approximately equal size for training and testing. The training and testing were performed using the 2-fold cross validation method. The detection performance of the CAD system was assessed by free response receiver operating characteristic (FROC) analysis. The average test FROC curve was obtained by averaging the FP rates at the same sensitivity along the two corresponding test FROC curves from the 2-fold cross validation. At the case-based sensitivities of 90%, 80% and 70% on the test set, our RDA classifier with the new feature selection scheme achieved an FP rate of 1.8, 1.1, and 0.6 FPs/image, respectively, compared to 2.1, 1.4, and 0.8 FPs/image with stepwise LDA with simplex optimization. Our results indicate that RDA in combination with the sequential forward inclusion-backward elimination feature selection method can improve the performance of mass detection on mammograms. Further work is underway to optimize the feature selection and classification scheme and to evaluate if this approach can be generalized to other CAD classification tasks.

Automatic and accurate segmentation of the pulmonary vessels in 3D computed tomographic angiographic images (CTPA) is an essential step for computerized detection of pulmonary embolism (PE) because PEs only occur inside the pulmonary arteries. We are developing an automated method to segment the pulmonary vessels in 3D CTPA images. The lung region is first extracted using thresholding and morphological operations. 3D multiscale filters in combination with a newly developed response function derived from the eigenvalues of Hessian matrices are used to enhance all vascular structures including the vessel bifurcations and suppress non-vessel structures such as the lymphoid tissues surrounding the vessels. At each scale, a volume of interest (VOI) containing the response function value at each voxel is defined. The voxels with a high response indicate that there is an enhanced vessel whose size matches the given filter scale. A hierarchical expectation-maximization (EM) estimation is then applied to the VOI to segment the vessel by extracting the high response voxels at this single scale. The vessel tree is finally reconstructed by combining the segmented vessels at all scales based on a "connected component" analysis. Two experienced thoracic radiologists provided the gold standard of pulmonary arteries by manually tracking the arterial tree and marking the center of the vessels using a computer graphical user interface. Two CTPA cases containing PEs were used to evaluate the performance. One of these two cases also contained other lung diseases. The accuracy of vessel tree segmentation was evaluated by the percentage of the "gold standard" vessel center points overlapping with the segmented vessels. The result shows that 97.3% (1868/1920) and 92.0% (2277/2476) of the manually marked center points overlapped with the segmented vessels for the cases without and with other lung disease, respectively. The results demonstrate that vessel segmentation using our method is not degraded by PE occlusion and the vessels can be accurately extracted.

We are developing a computer-aided detection (CAD) system to detect microcalcification clusters automatically on full field digital mammograms (FFDMs). The CAD system includes five stages: preprocessing, image enhancement and/or box-rim filtering, segmentation of microcalcification candidates, false positive (FP) reduction, and clustering. In this study, we investigated the performance of a nonlinear multiscale Laplacian pyramid enhancement method in comparison with a box-rim filter at the image enhancement stage and the use of a new error metric to improve the efficiency and robustness of the training of a convolution neural network (CNN) at the FP reduction stage of our CAD system. A data set of 96 cases with 200 images was collected at the University of Michigan. This data set contained 215 microcalcification clusters, of which 64 clusters were proven by biopsy to be malignant and 151 were proven to be benign. The data set was separated into two independent data sets. One data set was used to train and validate the CNN in our CAD system. The other data set was used to evaluate the detection performance. For this data set, Laplacian pyramid multiscale enhancement did not improve the performance of the microcalcification detection system in comparison with our box-rim filter previously optimized for digitized screen-film mammograms. With the new error metric, the training of CNN could be accelerated and the classification performance in validation was improved from an A_z value of 0.94 to 0.97 on average. The CNN in combination with rule-based classifiers could reduce FPs with a small tradeoff in sensitivity. By using the free-response receiver operating characteristic (FROC) methodology, it was found that our CAD system can achieve a cluster-based sensitivity of 70%, 80%, and 88% at 0.23, 0.39, and 0.71 FP marks/image, respectively. For case-based performance evaluation, a sensitivity of 80%, 90%, and 98% can be achieved at 0.17, 0.27, and 0.51 FP marks/image, respectively.

Automated registration of multiple mammograms for CAD depends on accurate nipple identification. We developed two new image analysis techniques based on geometric and texture convergence analyses to improve the performance of our previously developed nipple identification method. A gradient-based algorithm is used to automatically track the breast boundary. The nipple search region along the boundary is then defined by geometric convergence analysis of the breast shape. Three nipple candidates are identified by detecting the changes along the gray level profiles inside and outside the boundary and the changes in the boundary direction. A texture orientation-field analysis method is developed to estimate the fourth nipple candidate based on the convergence of the tissue texture pattern towards the nipple. The final nipple location is determined from the four nipple candidates by a confidence analysis. Our training and test data sets consisted of 419 and 368 randomly selected mammograms, respectively. The nipple location identified on each image by an experienced radiologist was used as the ground truth. For 118 of the training and 70 of the test images, the radiologist could not positively identify the nipple, but provided an estimate of its location. These were referred to as invisible nipple images. In the training data set, 89.37% (269/301) of the visible nipples and 81.36% (96/118) of the invisible nipples could be detected within 1 cm of the truth. In the test data set, 92.28% (275/298) of the visible nipples and 67.14% (47/70) of the invisible nipples were identified within 1 cm of the truth. In comparison, our previous nipple identification method without using the two convergence analysis techniques detected 82.39% (248/301), 77.12% (91/118), 89.93% (268/298) and 54.29% (38/70) of the nipples within 1 cm of the truth for the visible and invisible nipples in the training and test sets, respectively. The results indicate that the nipple on mammograms can be detected accurately. This will be an important step towards automatic multiple image analysis for CAD techniques.

An ROC study was conducted to evaluate the usefulness of assessing breast lesion characteristics with stereomammography. Stereoscopic image pairs of 158 breast biopsy tissue specimens were acquired with a GE digital mammography system. Two stereo image pairs were taken at 1.8X magnification geometry and at approximately orthogonal orientations for each specimen. Display software was developed for a high resolution MegaScan CRT monitor driven by a DOME stereo display board. The specimens contained either a mass, microcalcifications, both, or normal tissue. About 40% of the specimens were found to contain malignancy by pathological analysis. Five MQSA radiologists participated in the observer performance experiment. The two views of each specimen were read independently and were separated by a large number of other specimen images to reduce any effects of memorization. Each observer read 316 specimen images in a systematically randomized order. The observer first read the monoscopic image and entered his/her assessment in terms of the confidence ratings on the presence of microcalcifications and/or masses, margin clearance, BI-RADS assessment, and the likelihood of malignancy. The corresponding stereoscopic images were then displayed on the same monitor and were viewed through stereoscopic LCD glasses. The observer was free to change the ratings in every category after stereoscopic reading. The ratings of the observers were analyzed by ROC methodology. For the 5 MQSA radiologists, the average Az value for estimation of the likelihood of malignancy of the lesions improved from 0.70 for monoscopic viewing to 0.72 (p<0.05) after stereoscopic viewing, and the average Az value for the presence of microcalcifications improved from 0.94 to 0.95 (p<0.05). The Az value for the presence of masses improved from 0.80 to 0.82 after stereoscopic viewing, but the difference fell short of statistical significance (p=0.08). The visual assessment of margin clearance was found to have very low correlation with pathological analysis. This study demonstrates the potential of using stereomammography to improve the detection and characterization of mammographic lesions.

For the development of computer-aided diagnosis (CAD) systems, a classifier that can effectively differentiate more than two classes is often needed. For example, a detected object on an image may need to be classified as a malignant lesion, a benign lesion, or normal tissue. Currently, a three-class problem is usually treated as a two-stage, two-class problem, in which the detected object is first differentiated as a lesion or normal tissue, and, in the second stage, the lesion is further classified as malignant or benign. In this work, we explored methods for classification of an object into one of the three classes, and compared the three-class approach with the common two-class approach. We conducted Monte Carlo simulation studies to evaluate the dependence of the performance of 3-class classification schemes on design sample size and feature space configurations. A k-dimensional multivariate normal feature space with three classes having different means was assumed. Linear classifiers and artificial neural networks (ANNs) were examined. ROC analysis for the 3-class approach was explored under simplifying conditions. A performance index representing the normalized volume under the ROC surface (NVUS) was defined. Linear classifiers for classification of three classes and two classes were compared. We found that a 3-class approach with a linear classifier can achieve a higher NVUS than that of a 2-class approach. We further compared the performance of an ANN having three or one output nodes with a linear classifier. At large sample sizes, a 3-output-node ANN was basically the same as that of a one-output-node ANN. When the three class distributions had equal covariance matrices and the distances between pairs of class means were equal, the linear classifiers could reach a higher performance for the test samples than the ANN when the design sample size was small; the linear classifier and the ANNs approached the same performance in the limit of large design sample size. However, under complex feature space configurations such as the class means located along a line, the class in the middle was poorly differentiated from the other two classes by the linear classifiers for any dimensionality; the ANN outperformed the linear classifier at all design sample size studied. This simulation study may provide some useful information to guide the design of 3-class classifiers for various CAD applications.

A database of 145 mammograms containing biopsy proven malignant or benign microcalcifications was digitized with a laser scanner at a pixel size of 35 micrometer by 35 micrometers. Digitization at larger pixel sizes was simulated by averaging adjacent pixels. The individual microcalcifications were segmented from the digital images with region growing and adaptive gray level thresholding techniques. The characteristics of the individual microcalcifications were analyzed with visibility descriptors and shape descriptors. The variations of visibility and shape of the microcalcifications in a cluster were evaluated by the standard deviation, the coefficient of variation, and the maximum of each of the descriptors. In addition, texture features were extracted from the spatial gray level dependence (SGLD) matrices in the region containing the cluster of microcalcifications. A genetic algorithm (GA) was used to select features from the multidimensional morphological and texture feature space. Linear discriminant analysis was employed to classify the microcalcifications as benign or malignant based on the selected features using a cross-validation training and test method. The classification accuracy was evaluated by the area, A_z, under the receiver operating characteristic (ROC) curve. We varied the pixel size from 35 micrometers by 35 micrometers to 140 micrometers by 140 micrometers. The classification accuracy using morphological features decreased as the pixel size increased from 35 micrometers by 35 micrometers. In the texture feature space, the classification accuracy did not depend strongly on the pixel size. In the combined morphological and texture feature space, the classification accuracy also tended to decrease as the pixel size increased from 35 micrometer by 35 micrometer. The effect of spatial resolution on computerized classification depends on the features extracted for the classification task. We have not explored all possible features or feature parameters in this study and therefore the high spatial resolution information in the images may not have been fully exploited. Further studies to develop new features are being conducted and the impact of the spatial resolution needs to be further analyzed with a larger data set.

We are investigating the use of texture features to distinguish abnormal from normal tissue for computer-aided diagnosis algorithms. In this study, the improvement in the accuracy of classifying masses from normal breast tissue on digitized mammograms by using local texture features was evaluated. One hundred and sixty-eight mammograms were randomly selected from patient files and digitized with a laser scanner at a pixel size of 100 micrometers . Four different regions of interest (ROIs), each of 256 X 256 pixels, were selected manually from each of the digitized mammograms. One of the four ROIs contained a biopsy-proven mass and the other three contained normal parenchyma including dense, mixed dense/fatty, and fatty tissues. The mass ROIs were randomly and equally divided into a training and a test group along with corresponding normal ROIs from the same film. The wavelet transform was used to decompose the ROIs into several scales. Global multiresolution texture features were calculated from the spatial gray level dependence matrices of the low-pass wavelet coefficients up to a certain scale and then at variable distances between the pixel pairs. Texture features and their differences in suspicious object sub-region and its peripheral sub-regions of the ROIs were also calculated to form a local texture feature space. Stepwise linear discriminant analysis was used to select effective features from the combined global-local feature space to maximize the separation of mass and normal tissue for all ROIs. Receiver operating characteristic (ROC) analysis was used to evaluate the classification accuracy and its improvement using features from global and local feature spaces. Using the global multiresolution feature space alone, it was found that the texture features at large pixel distances were important for the classification task. Using local features only, the classification accuracy was comparable to that of the global features. With the combined global and local feature spaces, the average area, A_z, under the ROC curve reached 0.91 and 0.90 for the training and test groups, respectively. The improvement was statistically significant. The results demonstrate that a linear discriminant classifier using the combination of global multiresolution texture features and the local texture features can effectively classify masses from normal tissue on mammograms.
